We follow federal laws in regard to the disposition of your personal health information. PLEASE NOTE: We will not release any information from your Personal Medical File without your written and dated consent . On rare occasions (i.e., a serious threat to your health and safety, or the health and safety of another person, suspected child abuse, an authorized subpoena, etc.) we may have to release medical records. We will only share information with a person or organization that is relevant to your specific situation.

YOUR RIGHTS REGARDING YOUR HEALTH INFORMATION

You can ask us to call you at home and not at work to schedule or cancel an appointment.
You have the right to ask us to limit what we tell certain individuals involved in your care or the payment for your care, such as family members or friends.
You have the right to look at the health information, such as your medical and billing records.
You can request us to make changes (called amending) if you feel something may be inaccurate by placing it in writing and submitting it to our Privacy Officer.
You have the right to a copy of the Notice of Privacy form.
You can revoke an authorization to release personal health information at any time by contacting our Privacy Officer
